Yes. Also eye color is listed (despite there being a photo, making it seem kinda unnecessary). Also technically self-reported. So my legal eye color might be different than my actual eye color.

I don't check my passport often, so I just assumed it would have that same kind of info.

You can also change your reporting of eye color too. (possibly even height) I know I did when they darkened

That's so weird, we only have a name, date & place of birth, place of residence, black & white photograph and signature.

Isn't 4(c) supposed to be the issuing authority instead of your residence? Or is it a Belgian thing because your government is confusingly complex?

All the licences I've had in Europe were always just listing the issuing authority of the country in question. It would have been a bit painful to get a new licence every time I moved.
