Were you playing online? There was a bug a while back that could cause players in the same instance to all take damage if one player did.
Networking was enabled but I didn't have any company so far as I knew
So basically you are always online unless you specifically play offline.
Meaning you can encounter other players at the nexus and in other areas. It's just rare to find other players outside of the nexus unless you are doing weekend missions that use the same planet systems.
When I search "random damage no man's sky" it feels like people encounter crazy damage bugs that can even follow them into their ships. I wanna play permadeath with my best friend online. We've never had this problem while together but who knows what triggers it.
I hate to say it but I restored my save from a backup today and will probably restore for every time my own death doesn't make any sense :/
