He won 9 states including California. Early on he was outperforming Biden before all the other candidates dropped out and endorsed Biden, not long after that he dropped. So not sure which states he was on the ballot for.

Chicagoans can permanently register to vote by mail! Just fill out a form and the city will send you a ballot before every election! There's no reason to accidentally not vote anymore.
