It generally works if the government is unpopular enough and there's a parallel violent resistance movement. But when the civilians aren't stupid and they remember their neighbors being invaded by the US they understand that a regime change will only make things a lot worse. Plus, they understand that their economic grievances are completely manufactured by western illegal sanctions, and they built a medical industry to compensate for this (because people were dying a lot from lacking basic medicine).
