Any tips for hotbar management? (lemm.ee)

I'm diving into a second play of the game. I was okay with the hotbar system when first starting the game but it was a cluttered hot mess by the end.

Also, skills and spells not falling off the custom bar anytime I change equipment, get disarmed, disrobed, or walk into a stiff breeze is my #1 requested feature at the moment.

What's everyone doing to make their hotbars easier to manage?

For casters, I tried to keep concentration and non-concentration spells on different rows. And I try to keep abilities that recharge on a short rest on the left side of the bar to encourage me to use them
