Seriously, the point of a hot dog is being a finger food. There's no good way to do that here. Just cut up the dog and put it in the mac.
Hell yeah, grill that bad boy first, to pair with the smoked mac, then slice it up and top some mac with it. The restaurant could also decrease the food cost this way by decreasing the amount of hot dog and increasing the amount of pasta. The customer will still be happy, and you're probably saving 15-25% in food cost.
