Surely built before NYC had fire codes. In that case, do some buildings with "historical value" somehow get grandfathered in?

Yeah, at least in most cases. They get labeled as "legal non-conforming" and usually are required to become conforming in the event there is any major reconstruction done to fix a casualty event.

Depending on how the rooms are set up other side of the door. This maybe the best way to maximize pathing through the door. Swinging it other way round would put hinges and door in the way of moving..... You know furniture and stuff like a dresser or couch.or fridge.

This way its almost a straight shot through the door with minimal obstructions.

Also the photo looks distorted. Like fisheyed... perspective may be a bit off here.
