Yeah... definitely preheat your oven. They do not heat up in a linear fashion and putting your food early does not guarantee it gets cooked all the way
Don't think so. With my current stove I've been absolutely fine with the "target time plus five minutes" rule for everything, from thin pizza to large casseroles.
