Western anime fandom is defined in large part by porn addiction, and NTR is the preferred tag for that content on hentai sites.
I got the impression that the word wasn't widely known among the general population of Japan. I only heard about it through straight men coming into online Japanese language gay kink communities.
That wouldn't surprise me, it might be more well known in the West as a loanword than in Japan lol
