Trump may have a slight injury to his ear.
For a less conspiratorial take, here's a NYTimes archive / live version :

When the pops began at 6:13 p.m., Mr. Trump appeared to grab his right ear, which appeared to be bleeding as he left the stage.

Well that clears it up for me! There's just no way Trump would risk himself like that. This was real.
